Insurance

The City provides the ability for an employee to select benefits that best fits the needs of the employee and his/her dependents.  All coverages (except life insurance) are effective the 1st of the month following 60 days of continuous service. The group term life which is effective the 1st of the month following 30 days of continuous service.
 

Cafeteria Insurance Plan


The City provides each eligible employee a defined contribution of $200 per month ($100 bimonthly) called a cafeteria benefit. This may be used to offset the cost of the benefits selected such as dependent health, dental, vision (on a pre-tax basis) or disability and life insurances (on a post-tax basis). If you choose not to elect any voluntary benefits, this cafeteria benefit (defined contribution) is provided to the employee on a taxable basis. If this cafeteria benefit is waived, the City will pay 70% of the dependent health insurance premium. 

Health Insurance


Employees have a choice of three health plans: an HMO, a hybrid PPO, or a high deductible PPO health plan with an HSA (health savings account) through Florida Blue. The City pays 100% of the employee premium for the plan selected. Dependent coverage is also available. 

Disability Insurance


The City provides voluntary short-term and long-term disability paid by the employee through Florida Combined Life Insurance Company. The benefit is equal to 60% of the base salary.

Flexible Spending Account


The City provides an employee the ability to save pretax dollars in an account to be used for eligible medical or dental expenses and dependent day care expenses.

Life Insurance


The City provides term life insurance (including AD&D) at no cost to the employee. Life insurance is effective the 1st of the month following 30 days of continuous employment. The benefit is equal to 1 times the annual base salary. Also available is a voluntary term life policy paid entirely by the employee. This plan provides a benefit to employee, spouse and eligible children.

Dental Insurance


The City provides a choice in PPO dental plans (copayment or coinsurance). This premium is paid entirely by the employee at the following bimonthly deductions.

Vision Insurance


The City provides a network-based vision plan that emphasizes high quality routine eye health care from independent eye care professionals through Humana Vision.  The premium is paid entirely by the employee.

AFLAC Voluntary Benefits


The City provides choices of plans for cancer, accident and hospital benefits through AFLAC. These policies provide cash for specific services to help alleviate the financial burden associated with an injury or illness. The premiums are paid entirely by the employee.
